My services are designed for health educational purposes only. I am only acting as health and wellness coach. I cannot and do not provide specific treatment advice to anyone. I am not acting in the capacity of a physician, licensed dietitian-nutritionist, therapist, or psychologist and will not diagnose, treat or cure any disease, condition or other physical or mental ailment of the human body. You are responsible for your own health care decision-making and should obtain necessary consultations with appropriately licensed health care professionals such as physicians and psychologists before seeking any new treatment.

It is common practice for health coaches, nutritionists and other non-licensed practitioners to collect your signature on a liability waiver. By doing so you acknowledge that it is your responsibility to deliver all lab results to your own personal physician for any medical interpretation or opinion. My services only address a nutritional interpretation of test results that are used solely by you as an educational tool for personal health purposes. The liability waiver releases Patricia Nutrition from any liability for injury or loss arising out of the use of or reliance on the laboratory results and/or dietary, supplement, and lifestyle recommendations.

All lab test kits and dietary supplements ordered are billed at time of the order is placed. Consulting services are due at the time of service as outlined under IHP Service Rates and Packages section.

To reschedule or cancel a scheduled coaching session, please email me at least 24 hours before the planned call. And, if you know you will be more than five minutes late to a scheduled call, please let me know.
